Survey of Community Livability for Seniors

Several agencies in Clay County are partnering on a project called “Silos to Circles” with the intent of doing a better job of reaching out to seniors / older adults across Clay County and ensuring early intervention as seniors age to help them maintain their independence and promote quality of life.  The first step in this project is to complete a survey followed by focus groups to better inform our group of the needs within each of the seven targeted communities across Clay County.  Our goal is to distribute this survey to as many people across Clay County as possible through April 30th, 2018. Anyone can participate, because we understand that if you are not a senior / older adult yourself, you may know someone that would fall into that age group.

Please complete the Community Livability for Seniors Survey today.

Note:  Paper surveys are being distributed to Senior Centers, churches and agencies across Clay County. For a copy of a paper survey, contact Pat Boyer, Clay County Social Services at 218-299-7136 or e-mail: pat.boyer@co.clay.mn.us.
