Renewal Forms

All providers must renew participation in the Child Care Assistance Program (CCAP) every 2 years. To renew your CCAP registration you must complete the forms below and return them to the CCAP Provider Payment Contact. The renewal forms must be submitted to Clay County Social Services prior to the provider's renewal date:

• Licensed Exempt Provider Registration & Acknowledgement (PDF)
• Online Billing Registration (PDF)
• W-9 Request for Taxpayer ID and Certification (PDF)
