Child Care Assistance Program (CCAP)
Recipients
The Child Care Assistance Program (CCAP) can help families with low income pay child care costs for all children age 12 and younger and for handicapped children ages 13 and 14.
CCAP Provider Registration
The CCAP requires that a child care provider be registered with the county in which the family receiving child care lives to be authorized to receive child care assistance payments. Clay County accepts and processes registrations for providers who serve CCAP families residing in Clay County.
If you want to be authorized in more than 1 county, you must register with each county.

Renewal Forms
All providers must renew participation in the Child Care Assistance Program (CCAP) every 2 years. To renew your CCAP registration you must complete the forms below and return them to the CCAP Provider Payment Contact. The renewal forms must be submitted to Clay County Social Services prior to the provider's renewal date:
- Legal Nonlicensed Provider Registration and Acknowledgement (PDF)
- Online Billing Registration (PDF)
- W-9 Request for Taxpayer ID and Certification (PDF)
