Covid-19 Vaccine Clinic location: Family Service Center building, 715 11th St. N Moorhead (Use Main Entrance - East Side)
See links below to register for the Wednesday COVID Vaccine Clinic. If you are in need of an appointment for a different day or time, please call the CCPH Clinic at 218-299-7777 to schedule an appointment.
For those receiving a second dose or booster dose, please bring your vaccination card.
Moderna COVID Vaccine Clinic for 18 years and older
Offering primary series, first and second booster dose of Moderna. First booster Moderna dose available for those 18 years and older and must be five months after a second dose of Moderna or Pfizer or 2 months after a Janssen (J&J) dose.
Second booster Moderna dose available for those 50 years and older and must be FOUR MONTHS after first booster dose. For those 18 years and older who are moderate to severely immunocompromised, second booster dose must be at least 3 months after first booster dose or 2 months after second dose, if the first dose was J&J.

Pfizer COVID Vaccine Clinic for 12 years and older
Offering primary series, first and second booster dose of Pfizer. First booster Pfizer dose available for those 12 years and older and must be five months after a second dose of Pfizer or Moderna or 2 months after a Janssen (J&J) dose.
Second booster Pfizer dose available for those 50 years and older and must be FOUR MONTHS after first booster dose.
