Adult Health

At Clay County Public Health, we provide Care Coordination services to qualifying residents of Clay County. We make it our goal to support each individual giving them the support they need to achieve optimal well-being and thrive in their homes and community. We do this through providing comprehensive assessments to identify individual needs to deliver personalized, client-centered care. We aspire to ensure access to the essential services and supports to help members of our aging community live well.

Case Management

Clay County Public Health provides case management services for people eligible for MSHO / MSC+ / SNBC health plans, as well as those receiving Elderly Waiver (EW) / Alternative Care (AC) / Community Alternative Care (CAC) / Essential Community Supports (ECS).

Case management activities may be provided to or arranged for a person. Activities help people access, coordinate and monitor needed services as they relate to the person’s assessed needs and preferences. These include educational, medical, social and vocational services. Case Management activities may include, but are not limited to:

  • Coordinating services 
  • Developing a service plan based on people’s assessed needs and preferences 
  • Evaluating and monitoring services identified in the service plan 
  • Helping people access needed services 
  • Helping people identify potential service providers 
  • Informing people of their legal representative of service options


Long Term Care Services & Supports - MnCHOICES

MnCHOICES determines eligibility for publicly funded programs and services for all ages and disabilities including:

  •  Alternative Care (AC) program  
  • Brain Injury Waiver 
  • Case Management 
  • Community Alternative (CAC) Waiver    
  • Community Access for Disability Inclusion Waiver 
  • Consumer Directed Community Supports (CDCS)
  • Consumer Support Grant 
  • Developmental Disabilities Waiver 
  • Elderly Waiver (EW) program 
  • Essential Community Supports
  • Personal Care Assistance Services
  • Semi-independent living services

Vulnerable Adult Protection & Elder Abuse

If you suspect vulnerable adult abuse, neglect, or financial exploitation, please call Minnesota Adult Abuse Reporting Center (MAARC) at 1-844-880-1574.

In the event of an emergency that requires immediate assistance from law enforcement, fire department, or ambulance, dial 911 before contacting any other resource.

The Minnesota Department of Human Services provides information and services regarding Vulnerable Adult Protection & Elder Abuse, including reporting suspected maltreatment, as well as:
